Orthodontic treatment for

Patient was predominately concerned about the increased gum show on smiling at the back teeth. Patient presented with increased protrusion of the upper front teeth as well as an open bite at the front teeth. On smiling, there was remarkably increased gum show at the back teeth whilst at the front teeth she had the ideal amount gum and tooth-show on smiling and at rest. Patient had previously seen a few orthodontists and oral and maxillofacial surgeons who proposed double jaw surgery. Although, orthodontic treatment in combination with double jaw surgery would have been the ideal treatment, patient declined to undergo such treatment. We aimed at maintaining the size of the jaws whilst at the same time we wanted to; 1. Support the facial profile with any kind of orthodontics that we provide 2. Correct the open bite at the front teeth. 3. Reduce the gum show at the back teeth. 4. Maintain the ideal tooth and gum show at the front teeth.Whilst extraction of teeth would have been a very simple and straightforwards option to address the bite and protrusion of the upper teeth, in this case, it would have certainly had very detrimental effects on the facial profile. The proposed plan of the treatment entailed the intrusion the back teeth with a view to; 1. Reducing the gum show at the back teeth. 2. Correcting the open bite at the front teeth. Extrusion of the upper front teeth was avoided as patient already had the ideal tooth and gum show at the front teeth on smiling and at rest. The orthodontic treatment entailed upper and lower ceramic clarity 3M brackets and mini-implants. As the back teeth were intruded into the bone, the gum and tooth show at the back teeth on smiling was made ideal so that the gum margins follow the outline of the upper lip as much as possible on smiling. The intrusion of the back teeth also allowed for correction of the open bite at the front teeth without affecting the position of the front teeth. We did not want make any vertical changes to the position of the front teeth as we already had ideal incisor show on smiling.
